Just follow these steps and get your badge in no time!

   

Print out the Record Card to keep track of your progress. Fill in your name.
   

The badges can only be earned by boys before Bar-Mitzvah, and girls before Bas-Mitzvah. If you are 9 years old you must do at least step one to receive the badge. Ten or eleven, then you need step one and two . Eleven and older must do all three steps.
   
Fulfill each requirement and get it signed off by your parent (on the Record Card). Make sure they sign their name on the 'Instructors' line.
   
Print out the Registration Form, and ask your parent to fill it out.
   

Send the form (by fax or mail) with your filled out Record Card to Tzivos Hashem.
Make sure to include two dollars ($2) for shipping to the Tzivos Hashem Awards Program.
